We schedule appointments directly with the patient or the patient's caregiver (if under age 18). All additional information from current mental health and primary care physicians are welcome.
For Child and Adolescent:
Although we schedule appointments directly with the patient or caregiver, we require a physician, psychologist, or licensed mental health provider to provide information for all new patients.
Our Provider Information Form can be downloaded and then completed via computer entry or handwritten format. The completed form should be faxed to our clinic at 317-948-0609.
We also require that the patient's caregiver complete a Patient Information Form that can also be downloaded and completed via computer entry or handwritten form. The completed form may be faxed or mailed to our clinic.
We require the submission of both completed forms before we schedule the patient. The forms may be submitted separately.
